CSB Finds Corroded Sprinkler Triggered Catastrophic Bio-Lab Chemical Fire

The CSB found that a corroded sprinkler component allowed water to contact reactive pool-treatment chemicals, triggering the catastrophic Bio-Lab fire and toxic gas release.

The U.S. Chemical Safety and Hazard Investigation Board has released its final report on the September 2024 chemical fires and toxic gas release at the Bio-Lab facility in Conyers, Georgia.

The CSB determined that a corroded sprinkler-system component failed, allowing water to contact highly reactive pool-treatment chemicals stored in the warehouse. The resulting decomposition reactions generated intense heat, multiple fires, and a massive plume containing chlorine, hydrogen chloride, bromine, and other substances.

Approximately 17,000 people were reportedly evacuated, about 90,000 people were initially advised to shelter in place, and residents near the facility received recurring shelter-in-place warnings during the extended emergency response.

The investigation found that the warehouse contained nearly 14 million pounds of reactive chemicals—more than twice the inventory originally indicated to local officials. More than 1,100 sprinkler heads had been observed to be corroded during an inspection approximately nine months before the incident.

The CSB identified five major safety issues:

  • Operating equipment until it failed rather than correcting known deterioration
  • Failure to fully identify and control chemical-storage hazards
  • Inadequate risk management and corporate oversight
  • Gaps in industry guidance for pool-treatment chemicals
  • Limited regulatory coverage of reactive chemical hazards

Why this matters

This incident demonstrates how inventory growth, storage changes, corrosion, moisture, incompatible materials, and unresolved maintenance problems can interact. A warehouse may have procedures and fire-protection equipment on paper while changes in actual quantities, configurations, and operating conditions quietly undermine those safeguards.

Facilities storing reactive or water-sensitive chemicals should confirm that storage configurations, inventory limits, building systems, sprinkler materials, inspection findings, and emergency plans remain appropriate as operations change. Known deterioration should be treated as a warning requiring corrective action—not as a condition to manage until failure.
